Leslie Dale Schlake, 54, of Gothenburg, passed away on April 22, 2009, at his home in Gothenburg.
He was born on January 31, 1955, to John and Elaine (Schmidt) Schlake at Beatrice, NE. He graduated from Gothenburg High School in 1973 and Creighton Medical School in Omaha in 1982. He is a 1985 graduate from University of Iowa Family Practice Residency Program
Leslie worked as a Physician in Broken Bow for 5 1/2 year, was a Emergency doctor in Grand Island, Hastings and North Platte for 3 1/2 years and Owned Norfolk Primary Care in Norfolk for 12 years. He was a member of St. Johns Lutheran Church. In his spare time, he enjoyed cars and traveling.
He married Yvonne Franssen in 1980 and to this union, three children were born; Jeffery (Marjolaine) Schlake of Lincoln, NE, Erin Schlake of Norfolk, NE, and Lesley (Micah) Roberts of Norfolk, NE.
Leslie is survived by his children, parents John & Elaine of Gothenburg, NE; and a brother, LaMonte (Darla) Schlake of Gothenburg, NE; ex-wife Yvonne Schlake of Norfolk, NE.
A Memorial Service will be held in Norfolk at a later date.
